I've got a bunch of HTML 5 pages (from the HTML 5 plugin) and yesterday I was modifying the content of these quite successfully (i.e. I could immediately see the page being updated on my iPhone), and today... nothing updates. I make the change, press Save, but the changes are not updating now. I do get a message saying "This app's content has changed, would you like to refresh?", to which I click Yes. Also, I can see the correctly updated page directly in my browser, but the App is not refreshing these itself. I'm guessing it was the Custom URL as you can open a browser with the address? Could the app be caching the page? Try to "clean' the project and rebuild. Was a combination of ensuring it was set to Force Refresh = Yes, and when that partially worked (i.e. some pages refreshed and others didn't), then the ultimate solution was to delete the app from my iPhone and reinstall it.
