I am looking for a plugin that is a form that has multiple choice answers that can be saved and new answers added daily and preferably be put into some kind of chart to show how the answers have changed/stayed the same over a specific time (example a week, month, year). If you have something like this or want to make this please let me know how much it would cost. Thank you for any help you can provide.

Thanks CakeBit! Don't forget the Ultimate Screen Creator plugin - it can do multiple choice answers from a tableview as well. As for where the data gets inserted into, that's up to the php script you create to handle the data submissions to your server.
